Water-based lubricants that contain glycerin can cause diarrhea when used anally for some people who are sensitive to it. Try silicone lube or coconut oil. (Note that silicone lube cannot be used with silicone sex toys, and coconut oil cannot be used with condoms.)
Some people do have a sensitivity to semen, and it is possible this could cause diarrhea, but it is fairly uncommon. Is it possible that instead of diarrhea it is merely the mixture of semen and lubricant flowing out some time later in the day? It can stay up there for some time if not pushed out shortly after ejaculation.
Aside from a sensitivity to semen, there is no medical harm that can come from ejaculating inside her rectum (assuming you are STD-free, etc., of course).
